Background
Biofilms in dental unit waterlines (DUWL) are a potentially significant source of contamination posing a significant health risk as these may come into contact with patients and dental staff during treatment. DUWLs contaminated with Legionella pneumophila have caused death in Italy and Sweden. Another case saw two HIV patients develop dental abscesses associated with Pseudomonas, which was subsequently found in the DUWL.
The aim of the study was to evaluate the microbiological quality of DUWL water treated by Biofilm-Removal-System® (BRS®) and Alpron®/Bilpron® disinfectant solutions for six years in a French university hospital. The study focused on 68 dental chairs, all bottle fed and a mixture of old and new chairs. The initial water testing showed C.F.Us of over 300 per ml, the UK and EU standard for drinking water being less than 100 C.F.U. 7 units were contaminated with Legionella and Pseudomonas.
Shock treatment was performed on the chairs using the BRS® then continuously treated by Alpron® with sterile water during working days and Bilpron® during inactivity period, and combined with purging every morning and after each patient. In addition, the clean water bottles were disinfected daily in a washer disinfector.
The Results
The results of the study were quite incredible. Of 628 water samples taken over the 6 year period, there was only a single case of non-compliance to the drinking water standard. Additionally, 573 of the 628 water samples showed zero C.F.U. There was no detection of Legionella, Pseudomonas or Coliforms.
The protocol implemented in our university hospital gives excellent results and enables control of the microbiological quality of DUWL water in the long term.
